If you are welcoming a non-patient-related visitor, there’s a new process to get their information to our team. Just open this Qualtrics survey, fill in the required fields, and you’re good to go!
Please be sure to complete the survey at least 24 hours in advance of the visit date, and please help us continue to demonstrate COVID-19 safety practices by limiting these visits to only those who need to physically be onsite.
Categories of non-patient-related visitors:
- Vendors
- Professional guests (i.e., meeting attendees, donors, etc.)
- Interview candidates/recruits
- Research participants
- New hires
- Graduate students attending class instructed by UI Health Care leaders
The patient visitor process continues to be managed through Epic.
