Honoring Your Wishes™ is a community-wide advance care planning initiative. Its mission is to promote an advance care planning process to help ensure that people’s health care preferences are discussed, documented, and honored by friends, family, and the health care community. University of Iowa Health Care is proud to be a community sponsor and partner.
The Community Champion award is given annually in memory of Dianne Day, who volunteered at the Iowa City/Johnson County Senior Center. Dianne facilitated over 100 advance care planning discussions with community members in 2011 through 2013 and wrote an editorial on advance care planning that was published nationally.
Each year a sub-committee of previous award winners and/or volunteers from the Honoring Your Wishes Community Engagement Task Force will select an individual or individuals for this award. Criteria include a volunteer or professional within an organization or in the community who:
- Volunteers talents and skills to promote the mission and vision of Honoring Your Wishes within their respective organization or throughout the community in significant ways and/or
- Provides leadership in promoting advance care planning best practices and improving systems and/or
- Sets the example for integrating advance care planning best practices within his/her clinical work
Nominations may be made by anyone in the community. Nomination deadline is Aug. 31, 2017. To nominate someone, please send a letter of support to Jane Dohrmann at email@example.com. The award will be given at the Honoring Your Wishes professional conference on Friday, Oct. 20, at the University of Iowa College of Public Health.
