New care coordination division enhances collaboration

In today’s health care environment, close staff collaboration is crucial. UI Health Care social workers and nurses have a long history of working together to provide the best and most efficient patient care.

We have now formalized that collaboration by creating a new care coordination division. This new division falls under nursing leadership. It includes nurse navigators, social workers, spiritual services, pediatric continuity of care, and more. All of these groups are coming together to decrease length of stay and improve patient satisfaction.

This administrative action brings UI Health Care in line with most large hospitals, which have combined social work and care management departments.

For front-line employees, little will change. Some people will see a change in their reporting structure. Offices may move for closer physical and professional collaboration.

We thank all the employees involved for their continued focus on exceptional patient care. More information will soon be available on The Point.
